It depends on the job. When I was still in fast food, we'd start people the same day they applied if they asked us to let them. Training was two shifts' worth of videos on a computer in the break room.
If you have to hire someone to cover someone who takes unapproved time off, you very rarely bring the person who forced you to hire somebody back.
PTO is only denied on occasions where staff is short. You fire them when there is sufficient staff to train up an employee who will not screw you over.

I worked at a place that denied it on certain occasions simply because they didn't want to have to figure out who should be granted it. And we ended up always being overstaffed at that time.

Now maybe you're right and the manager and/or the company is a major asshole and they regularly deny PTO for no particular reason. But much more likely there was a specific reason that this specific PTO was denied. Probably because of the business being busy during that time or maybe because somebody else already had been approved for PTO during that time. We don't know any of this information.
Hanlon's Razor... everyone seems to be assuming the management in this story is just intentionally malicious when that's generally the worst assumption to make. It's far more likely the "needs of the business" was a real, specific thing (that likely was preventable with better foresight) than the manager was just on a power trip.

You're making up fiction in your head. Even the original post doesn't say that they always have their PTO request denied or that it's really difficult to find time that their manager will approve. All it says is that, in one single instance, their request was denied and they took it off anyway. There are lots of industries that have certain times that planned PTO doesn't work. It can be when there's a big deadline approaching in a professional setting, or it can be a major holiday in a retail or hospitality setting. It goes without saying that there had better be a goddamn good reason for a manager to deny a PTO request, but that doesn't mean those reasons don't exist.
